Birth Certificate and Evidence of Name change
Hi all, My CO contacted me today, ask for certified copy of birth certificate but i don't have birth certificate.. What to do now..???
I added my surname in 2014 along with my name now he also required Evidence of name change. I only have copy if newspaper where i advertised about name change.
Can someone help me with this please that would be great.

U need statuary declaration for name change along with attested copy of newspaper.
If possible also add copy of an affidavit that was used for changing the name in overseas otherwise ask ur parents to give write a new affidavit declaring that u changed ur name in 2014.
